Insurance

As a convenience to you, our staff will process insurance claims for you. Please bring all necessary forms with you on your initial visit.

Generally, insurance policies only cover a portion of the total treatment cost. Therefore, we ask that any remaining balance from your services be paid within an allotted time that we will set for you. 

It is your responsibility to pay any balance that is not covered by your insurance company. We accept most insurance plans, including those listed below. 

We accept Visa, Mastercard, checks and cash. Our staff will also be glad to inform you about financing payment plan options that we can offer in-house or through Chase with 0% interest rate.
